Log Sheets

Your clinical progress in Pediatric Dentistry will be tracked through the use of log sheets.

The first log is the Attendance Log Sheet, which records your participation in the clinical care of children. You may request your clinical instructor (faculty or resident) to sign this at the end of each clinical session.

The second log is the Competency Exam Log Sheet, which records your progress toward the completion of competency examinations. You must inform the clinical instructor of your intent to take the competency exam before beginning the procedure. The instructor will sign if you have completed the procedure competently and with minimal assistance.

The Attendance and Competency Exam Log Sheets are to be turned in to Pat Stafford in CHS 23-022 by 4:00 PM on the Friday of the last week of instruction every quarter of your 4th year. These will be returned to you at the beginning of the next quarter.

The third log is the Procedure Log Sheet, which records your activity in the clinical care of children. No instructor is required to sign this. Demonstration of clinical activity on this log sheet will be according to the honor system.

The Procedure Log Sheet is to be turned in to Pat Stafford in CHS 23-022 by 4:00 PM on the Friday of the last week of instruction every quarter of your 3rd and 4th years. These will NOT be returned to you .
